I authorise REZ Headlight Restorations to perform exterior headlight restoration on the vehicle listed above. I understand that the service may include cleaning, surface preparation, multi-stage sanding, machine polishing and ceramic UV protection. Results depend on the existing condition of the headlights, and internal haze, condensation, cracking, crazing, deep damage or deterioration inside the headlight assembly may not be corrected by exterior restoration.

Photo & Video Consent

May REZ Headlight Restorations use before/after photos or video of your vehicle for marketing purposes?

If permission is given, photos or videos may be used on the REZ Headlight Restorations website, Google Business Profile, Instagram, Facebook, TikTok, printed marketing material and digital advertising. Registration plates and identifiable people will be cropped, blurred or obscured where reasonably possible.

This permission is optional and is separate from service authorisation. Leaving it unanswered is never treated as permission.
